Can Invisalign Fix Severe Bite Problems?

In Australia, Invisalign has transformed the landscape of adult orthodontics, but the question patients with severe bite problems are asking is: Can clear aligners actually do what traditional braces do? And the answer is not always as straightforward as you might expect. More than just a cosmetic concern, severe bite problems such as deep overbites, underbites, crossbites, and anterior open bites are among the most common oral health problems worldwide, according to the World Health Organisation, second only to tooth decay and gum disease.

If left untreated, severe malocclusions can lead to uneven tooth wear, temporomandibular joint disorders, speech problems, and decreased chewing efficiency. The right answer depends largely on what type of bite problem is being treated.

What the Research Actually Says?

In 2018, a systematic review in Progress in Orthodontics reviewed 22 clinical studies on Invisalign outcomes (randomised controlled trials, prospective studies and retrospective analyses) that concluded that Invisalign works predictably for mild to moderate malocclusions. It has significant limitations in treating significant vertical corrections, large anteroposterior movements, and complex occlusal relationships. Many of the studies in that review specifically showed lower predictability when treating major overbite discrepancies. The final outcome did not always correspond to the digital treatment simulation the patient was shown at the outset.

The difference between simulation and outcome is important, because it sets patient expectations before treatment starts. The digital planning tools used by Invisalign are advanced enough to model almost any movement, leading to the perception that the system can provide whatever the simulation shows. The clinical evidence suggests otherwise for the most challenging cases. The biological and mechanical limits of removable aligners have not disappeared. Knowing those limits is what differentiates a good match from a suboptimal treatment plan.

Where Invisalign Performs Well, And Where It Does Not?

Outcomes are quite variable depending on the type of bite problem, but the evidence for open bite correction is particularly promising. There is measurable bite closure for Invisalign patients (2023). Retrospective studies of 50 open-bite cases showed an average correction from a pre-treatment open bite of -3.5 mm to a positive overbite of +1.5 mm (statistically significant).

The Story with Crossbites Is Different

When the cause of a dental crossbite is primarily tooth position, aligner therapy can be very successful. When the problem is the width of the jaw itself, or a severe skeletal crossbite, fixed appliances, skeletal expansion, or orthognathic surgery is often needed. No amount of refinement can address a discrepancy that is at the skeletal, not dental, level. This is the most important distinction to make in severe cases. It is one that is easiest to miss if the conversation remains focused on the appearance of the aligner rather than the nature of the problem.

Compliance Is Not A Minor Variable

Invisalign is only effective with 20 to 22 hours of daily wear. Unlike fixed braces, which apply force continuously regardless of what the patient does, removable aligners stop working the moment they come out. Inconsistent wear will not only slow the process but can prevent the outcome in severe bite cases where the planned movements are already at the edge of what aligner therapy can reliably achieve. One reason for this is that research comparing planned and achieved tooth movements has demonstrated that rotational corrections, root control, and extrusion movements achieve lower accuracy than tipping movements, even under good compliance conditions.

Orthodontists will often add attachments, elastics, and temporary anchorage devices to make the Invisalign system more predictable for the most demanding movements. The additional components make the treatment more involved than the patient may expect when they initially ask about clear aligners.

When Braces Remain the Stronger Choice?

Fixed appliances are still preferred over aligners for severe skeletal discrepancies, pronounced jaw asymmetries, and the most complex multidimensional bite corrections. Braces apply continuous force and allow clinicians more control over root movement. This is essential when multiple axes of movement are necessary at the same time. In Australia, comprehensive Invisalign treatment for complex cases typically costs between AUD 6,000 and AUD 12,000. When surgery or extensive refinements are included, costs become astronomical. In a case that would eventually need braces or surgery anyway, starting with aligners simply costs more and takes longer. There is no benefit to the final outcome.

Any patient who suffers from a severe bite problem should begin with a comprehensive clinical assessment, preferably with three-dimensional imaging, before any treatment decision is made. Invisalign is truly effective for a wide range of conditions, and the technology keeps advancing. The capability to treat mild to moderate cases does not automatically translate into the capability to treat severe cases. The patients who fare best are the ones whose treatment choice was aligned with the actual nature of their problem rather than with the format they had preferred prior to the assessment.
